With printed conditions and instructions on the verso, two original signatures of the vendor and issuin clerk (3.) Form letter to Jacob M. Hoskins (doctor-vendee) with typescript conditions of approval and stipulations for alcohol use. Dated Dec 1, 1927, stamped by W. H. Walker and signed by a prohibition clerk.
